Field Trip Reservation Form
​Ready to visit Leonardo's Children's Museum? Field Trip reservations are first come, first serve and MUST be made a minimum of two weeks before planned visit so we can schedule adequate staff to make your visit as fun and safe as possible. Please complete the form below to submit your request.

We will review and contact you to confirm. If you do not receive confirmation from us within 72 business hours of submitting online reservation form, please contact us at (580)233-2787. We look forward to seeing you!

There have been recent changes to our field trip policies and procedures. Please read ​carefully!
    PLEASE READ: We require a ratio of 1 adult chaperone (16 or older) to 5 children. Children must be divided into a maximum group of 5 and accompanied by their adult chaperone at all times while in the museum.

    If you do not have the required ratio of adult chaperones to children, you will not be admitted.

    IMPORTANT: Cash or check payments must be received at least 1 week prior to your visit. Title 1 scholarships or purchase order payment terms must be established upon reservation.

    Payment should include admission for all adult chaperones as well as children. 
    ​This is done to makes your groups arrival at the museum as smooth as possible and to allow the fun and learning to start as quickly as possible!

    PLEASE READ
    • Free Passes “DO NOT” apply to school group visits. This is what allows us to give a discounted rate for these groups to attend and enjoy a visit to Leonardo's. We appreciate your understanding. Memberships are also excluded for students, but may be used by adult chaperones.
    • Price - $8.00 each for students and chaperones, if chaperons are using their membership, they must have their card with them or a photo of the front and back of their card for easy viewing.
    • Bus Driver is FREE
    • All groups must be divided into groups of 5 or less with at least one adult chaperone assigned to stay with them throughout the visit. Check in will be quicker if groups are assigned to chaperones prior to arrival at the museum.
    • About 24 hours before the visit you’ll receive courtesy call to verify filed trip details. This is a great time to ask questions about directions to the museum, bus parking, or any other details regarding your field trip.
    • No food or drink allowed on the museum floor, eating areas will be available.
    • Adventure Quest is open year around weather permitting!
    I understand the policies and procedures for field trips at Leonardo’s Children’s Museum. I understand as the group organizer that I am responsible for communicating to other teachers and parents about Leonardo's policies and procedures, especially regarding supervision of children. I am responsible for payment for each student and each chaperone for my group. My group will be given a color wristband to identify our school with and anyone not wearing the wrist band will not be considered a part of our group. Fees must be collected for students and chaperones in advance and paid 1 week before the field trip date. We must have this signed contract on file before the field trip is scheduled.

    If you arrive without the required 1-5 adult chaperone to child ratio, YOU WILL BE DENIED ENTRY.
